So you want to be recruited to be a member of the Volts, eh? *looks you up and down* I just need to clarify a few things with you.

You must be a VOLTS fan, at the very least, a GetBackers fan. Otherwise, what are you doing here? ~.^
You must give your name. But if you have a past you're desperately trying to escape from, an alias or nickname is alright, too. ^^
You must have a valid e-mail address. How else are we going to contact you? You know very well Ginji-san has no sense of direction! *looks around in case she's overheard*
You must state where you are from or where you're currently living. We might have to contact you when we need to round up the members for help in beating the bad guys.
It's fine if you don't have a website. Maybe we'll just ask Makubex to make you one. But if you do have a site and if it has anything offensive on it like racism, hate, porn, etc. then just don't include that in your info, your email will do fine. And the code should be up BEFORE I check your site.
